Do not leave the scene until you have collected all necessary information. Provide only the necessary and requested information to police and other parties involved in the accident. Do not discuss your personal condition, frame of mind, or anything unrelated to the accident. Use this Accident Information form. If you can, while the information is fresh in your mind, use the Accident Diagram to record how the accident occurred. This will help you later when speaking to the insurance company.
